Beneath it, leave a hand's width of table and lay four cards in a row from left to right, week one under your left hand and week four under your right. The gap matters: the theme belongs to all four weeks and should sit over none of them in particular. If you read for someone else, sit across from them so the row runs left to right for them too, like a calendar on a wall.

Her work in November is to notice which ships come back, meaning which referral sources and which kinds of patient, and to prepare to receive them before she sends out more.
The Knight of Swords rides in full course under racing clouds, sword raised, and reversed his charge has lost its aim. In week one this is the urge to do everything at once: messages to every doctor she has ever worked with, prices changed twice before the first patient walks in. Waite gives imprudence and extravagance for the reversal. Rein the horse in. Rest taken now costs far less than rest the body takes for her in December.
Two young people pledge cups of the same size at the same height, and above them the winged lion's head rises over the serpent staff. In week three a partnership arrives on fair terms, most likely an orthopaedic surgeon or a sports coach who sends patients and wants to hear how they are doing. The card's whole teaching is balance, so she should give as much as she hopes to get: a short written note back after every referred patient. This is the card that turns the theme's distant ships into actual arrivals.
A hand from a cloud holds out a single gold pentacle over a tended garden, and a path runs through a flowering arch toward the mountains. For the last week this is a first solid return: patients booking a second visit, and takings that cover the month's rent. Waite reads gold and contentment. An Ace is a seed, so before December begins she should write down exactly what came in and decide where it goes, one sum for the clinic and one kept aside.
